WINTER CLINICS TERMS AND CONDITIONS
For Registered Attendees
The Winter Clinics Committee maintains a commitment to the safety and health of our attendees while the COVID pandemic continues. Therefore, it has decided for the 2023 conference:
1) We highly recommend conference attendees, exhibitors, and sponsor representatives and guests appearing at Winter Clinics in person be fully-vaccinated against COVID-19 and Influence using an FDA approved vaccine. However, we are not requiring this. Guests must accept this risk.
2) All attendees should consider any recent "unmitigated exposures"* to COVID+ or persons with the influenza and/or their personal health status just prior to coming to the event. If a person must self-quarantine or stay home due to illness, we will refund you down to the online price for the registration ticket you chose. No full refunds will be provided. You must make this request prior to the start of the event February 24, 6 PM.
3) If you are coughing and/or sneezing or showing signs of respiratory illness, we kindly request for you to watch online from your room and/or mask while in public.
*By "unmitigated exposures" we mean following the normal public CDC guidelines for when to quarantine posted on their website. Their guidelines don't take into account healthcare workers who may be exposed routinely but mitigated by full PPE precautions.
These safety protocols may be strengthened or loosened based on prevailing conditions closer to the event. The event may also be cancelled or converted to a virtual event should extenuating circumstances occur.
Cancellation Policy
Self-initiated cancellation requests must be made in writing. For all other reasons besides a COVID-19 exposure and recommended quarantine, or isolation, they will be honored according to the following dates:
Late Registration Fee
Regular registrations made on or after February 11 are subject to a $35 surcharge, other than late banquet ticket purchases.
Use Agreement and Use of Likeness
By participating in Winter Clinics, you agree that ACMS may record your image and/or voice during the event broadcast. Recordings may be rebroadcast as ACMS sees fit without your permission or compensation. Please consider confidentiality when discussing any patient cases during the conference.
Disclosure Statement
None of the speakers, members of the planning committee, nor any member of their immediate families, have a relevant financial interest in or affiliation with any commercial supporter of this educational activity and/or with the manufacturer(s) of commercial products and/or providers of any commercial services discussed in this educational activity.

Accreditation Statement/Credit Designation
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Utah Medical Association (UMA) through the joint providership of the Southwest Idaho AHEC and Ada County Medical Society. The Southwest Idaho AHEC is accredited by the Utah Medical Association to provide continuing medical education for physicians. The Southwest Idaho AHEC designates this live activity for a maximum of 12.5 AMA PRA Category 1 Credit(s)TM.
Certificate of Attendance
Upon completion of a CME evaluation, you will receive a Certificate of Attendance based on actual attendance, whether in person or virtual. Failure to complete an evaluation within the time frame provided by SW Idaho AHEC (our accreditor) will result in denial of CME credit for your participation. This certificate It is intended for physicians, and can typically be used for equivalent credit by PAs, NPs and pharmacists. Other allied health care professionals will need to check with their national organizations to see if they will accept AMA PRA Category 1 Credit(s)™ towards licensure or certification.
AAFP Credit
ACMS is applying for credit through the American Academy of Family Physicians. Credit pending.
